Vital Statistics

The Florida Department of Health in Bradford County (DOH-Bradford) can assist you with obtaining a State of Florida birth certificate.

Birth Certificates are available for persons who were born in the State of Florida only from 1930 to the present. Birth Certificates earlier than 1930 can be obtained from the State Vital Statistics Office in Jacksonville by calling 904-359-6900 Ext. 9000 or by calling the credit card line at 877-550-7330.

Applying for a Birth Certificate

You may apply for Birth Certificates in person or by mail.

In Person: Our office is located at 1801 N Temple Ave, Starke, FL 32091. The vital statistics office is open from 9 a.m. - 4 p.m., Monday - Friday.

By Mail: Print and mail the completed Application for Birth Certificate form along with payment and a copy of appropriate identification to the DOH-Bradford Office of Vital Statistics, 1801 N Temple Ave, Starke, FL 32091.

Identification: Birth Certificates are NOT public record and may be issued to only certain individuals. Applicants for these certificates must present an official, picture identification card, such as their Driver's License, Passport, or other State Identification card.

Cost and Payment: Cash, Credit Cards, Money Orders, and Cashier's Checks are accepted. NO Personal Checks are accepted.

  • First Copy - $15.00
  • Additional copies ordered at the same time - $8.00 each
  • Protective Sleeve - $3.00

Restrictions

Birth Certificates can be issued only to the registrant (the child named on the record) if of legal age, parent(s) (name(s) that are stated on the birth certificate), guardian or other legal representative, or by court order.

DOH-Bradford Vital Statistics Office does not handle marriage licenses, divorce decrees, probate, wills, or Social Security.
